What is revision rhinoplasty (secondary nose surgery)?

Revision rhinoplasty is performed when, for various reasons, the desired results from previous rhinoplasty procedures have not been achieved. With revision rhinoplasty, the results of rhinoplasty that did not fully achieve their goals are reviewed and corrected.

When are revision rhinoplasty surgeries needed?

In rhinoplasty surgeries, 10-15% of cases may experience undesirable outcomes for both the patient and the surgeon 9-12 months later. These negative outcomes may necessitate a second revision rhinoplasty. The situations that necessitate revision rhinoplasty (secondary rhinoplasty) are as follows:

  • Breathing problems occur when the nostrils and internal structures narrow or close as the patient takes a deep breath, preventing insufficient air from entering the nose.
  • Aesthetic deformities and irregularities resulting from insufficient correction of the hump and curvature on the bridge of the nose during surgery.
  • Problems such as collapse, curvature, and irregularities in the nasal bridge due to a disruption in the relationship between the bone and cartilage structures on the nasal bridge.
  • Nasal deviation, or curvature of the nose, occurs when inadequate interventions on the bony structures forming the side walls of the nose cause the nasal axis to shift sideways from its proper midline.
  • Despite undergoing rhinoplasty, the nasal tip remains aesthetically unchanged. The drooping of the nasal tip and cartilage deformities persist.
  • Aesthetic problems such as excessive lifting and rotation of the nasal tip, and the inner nostrils being undesirably visible.
  • The patient has a problem with disproportionate size between the nose and face. This is due to inadequate aesthetic planning in a previous rhinoplasty, resulting in the nose being either too small or too small compared to the face.
  • Asymmetries in the nostrils, or the nostrils being excessively wide or excessively narrow.
  • Appearance problems resulting from an improper calculation of the harmony and angle between the nose and the lips.

What should be considered before surgery?

Nail polish, jewelry, and contact lenses should be removed before surgery. Birth control pills should be discontinued 2 weeks prior to surgery. Patients are advised to quit smoking approximately 2-3 weeks before the operation. Blood-thinning medications and herbal teas should be discontinued. Patients should inform their doctor of any medications they need to take during their appointment so that the treatment plan can be adjusted accordingly.

When can revision rhinoplasty (secondary rhinoplasty) be performed?

It is recommended to wait approximately a year after the initial surgery before considering a revision procedure. This is because it can take about a year for the nose shape to settle after rhinoplasty. Therefore, it is advisable to wait for this period to pass, and non-surgical methods should be preferred for minor corrections when necessary.

How long does revision rhinoplasty take?

The duration of this surgery varies depending on the condition of the nose, the patient’s current state, and the results of previous surgeries, but it takes approximately 2 hours.

Is revision rhinoplasty difficult?

Approximately 10-15 out of every 100 rhinoplasty surgeries may require revision surgery. Revision rhinoplasty isn’t always difficult, but it requires careful planning. Furthermore, it’s crucial that these surgeries are performed by experienced surgeons.

Secondary Rhinoplasty Techniques

Fearing and avoiding secondary rhinoplasty will not help you solve existing nasal problems. It should be remembered that any deformity in the nose will worsen in the long term and become even more difficult to correct.

Furthermore, secondary rhinoplasty operations are not always very complicated. In some cases, problems can be corrected with filler techniques. Of course, in some cases, surgery under general anesthesia is necessary.

However, regardless of the circumstances, it should be remembered that the nose is a very important organ both in terms of appearance and quality of life, and its problems must be addressed.

Secondary rhinoplasty procedures can be performed with minor fillers, or they can be comprehensive operations requiring cartilage tissue transfer. Additional cartilage is usually obtained from the ear.

Especially in cases where there is excess cartilage in the nose, the likelihood of needing additional cartilage after a first operation is high. In cases where cartilage taken from the ear(s) is insufficient, cartilage can also be taken from the ribs for the operation. Problems in the bone or cartilage framework are thus resolved.
