It is chosen mainly to change breast shape and position rather than breast size, and planning depends on an individual examination.

What is a breast lift (mastopexy)?

A breast lift, known medically as mastopexy, tightens the skin envelope of the breast, reshapes the breast tissue beneath it and repositions the nipple-areola complex so that it sits higher on the reshaped mound. The areola may also be reduced in diameter when this is clinically appropriate.

Mastopexy primarily addresses shape and position. Breasts may look fuller after reshaping because tissue is redistributed and the skin is tightened, but volume that has been lost over time is not necessarily restored by lifting alone. Patients who want a clear change in size usually need to discuss an additional procedure.

Suitability is determined during an in-person assessment that considers breast anatomy, skin quality and elasticity, breast volume, nipple position relative to the breast crease, medical history and the patient’s own goals. Two people with similar photographs can need quite different surgical plans.

When is a lift considered for sagging breasts?

A breast lift is generally considered when the nipples point downwards or sit at or below the breast crease, when the skin envelope has become loose after a change in volume, or when the breast tissue has descended enough to alter breast shape. Breast asymmetry, tubular breast shape and differences in areola size or position often need individual planning, since one breast may require more skin removal or a different nipple position than the other.

Breast ptosis, commonly described as breast sagging, develops when the skin and the internal supporting tissues stretch, so that the breast tissue descends and the nipple sits lower on the chest. The degree of descent, rather than breast size, is what defines ptosis.

Several factors contribute. Pregnancy and breastfeeding alter breast volume and skin tension; weight gain followed by weight loss stretches and then empties the skin envelope; ageing reduces skin elasticity; and gravity acts continuously on breast tissue over the years. Inherited skin characteristics also influence how much support the tissues retain. Bra habits are often mentioned in popular sources, but they are not established as a primary cause of ptosis.

An examination is needed to assess the degree of ptosis and to determine whether a lift on its own is appropriate or whether a combined procedure would suit the patient’s goals better.

Breast lift without implants

A breast lift without implants raises and reshapes the breast using the patient’s existing tissue, which can improve contour and nipple position but may not replace volume that has already been lost. The upper part of the breast, in particular, often remains softer than some patients expect.

This approach tends to suit people whose main concerns are a low nipple position, a loose skin envelope and a flattened contour, rather than a noticeable increase in size. The result depends on how much breast tissue is available to reshape and how the skin responds during healing.

When loss of volume or upper-pole fullness is the central concern, breast augmentation may be discussed alongside or instead of a lift, with its own set of considerations and risks.

In other cases the weight of the breast itself contributes to skin stretching, shoulder or back discomfort and recurrent sagging. For these patients breast reduction combined with lifting may be a more logical plan. No technique can promise a particular cup size, cleavage shape or a permanent result, since breasts continue to change with weight, hormones and time.

What happens during breast lift surgery?

Breast lift surgery is usually carried out under general anaesthesia and commonly takes around 2–3 hours, although the duration varies with the chosen technique and whether another breast procedure is performed at the same time. Many patients go home the same day or the following day.

The breast is marked while the patient is standing, so that the planned nipple position and the skin to be removed are set against the natural breast crease. Excess skin is then removed, the breast tissue is reshaped and secured in a higher position, and the nipple-areola complex is moved upwards on its own blood supply. The areola may be reduced in size if required. Incisions are closed with sutures, surgical tape or skin adhesive, and small drains are occasionally placed to prevent fluid collecting within the breast.

Preparation begins well before the operating day. Medical history, family history of breast cancer, current medications and supplements, smoking and alcohol use and previous breast surgery are reviewed, and blood tests or radiological breast imaging may be requested. Stopping smoking and avoiding alcohol is generally advised 2–3 weeks before surgery, while aspirin, vitamins, herbal medicines and herbal teas that can increase bleeding are usually stopped beforehand for a period that varies according to the surgeon’s instructions. Maintaining a stable weight and discussing expectations openly also form part of planning.

Like any operation, mastopexy carries risks that cannot be removed entirely. These include bleeding, infection, delayed wound healing or wound separation, fluid collection, changes or loss of sensation in the nipple or breast skin, asymmetry in shape or size, unfavourable scarring, circulation problems affecting the nipple-areola complex, and possible effects on breastfeeding.

Breast lift scars

Breast lift scars are an expected part of mastopexy, because skin has to be cut, removed and re-draped in order to raise the breast. What varies is the pattern and length of the scar, which follows the technique selected for the degree of sagging and the amount of reshaping required.

Scars may run around the border of the areola, vertically from the areola down towards the breast crease, and in some plans horizontally along the crease itself. No single pattern suits every patient; a limited scar is only possible when the amount of skin to be removed is correspondingly limited.

Scars usually change in colour and texture over many months and may remain visible even once they have matured. Skin type, individual healing tendency, wound tension, infection, smoking and sun exposure all influence the final appearance, and some people are prone to thickened or raised scars regardless of technique.

Practical care matters: following wound-care instructions, wearing the recommended support bra, keeping healing scars out of direct sunlight, and raising persistent, itchy or raised scars at follow-up appointments so that they can be assessed and managed.

Breast lift recovery

Breast lift recovery usually begins with swelling, tightness, bruising, discomfort and temporary changes in nipple or breast sensation, with the pace differing from person to person. Pain is most noticeable in the first few days and is generally managed with the painkillers recommended by the surgical team.

Most patients return to desk-based work after about 5–7 days, while those with physically demanding jobs or a regular sports routine may need 1–2 weeks. Swelling settles gradually over a period that varies from person to person, and numbness in parts of the breast or nipple can persist for some months before gradually improving. The breasts are usually supported with a surgical or sports bra, waterproof dressings may be used over the incisions, and any drains that have been placed are removed according to the team’s instructions.

Certain precautions help healing. Sleeping on the back avoids pressure on the reshaped breasts; heavy lifting, strenuous exercise and chest-focused training are postponed until cleared; friction or pressure over healing incisions is avoided; sexual activity is resumed only when advised; and healing skin is protected from sun exposure.

The final shape of the breast settles gradually as swelling resolves and tissues soften, and scar maturation continues long after the breast looks settled. Worsening rather than improving pain, fever, unexpected or foul-smelling drainage, rapidly increasing swelling on one side, or a dusky colour change in the nipple or skin should prompt contact with the surgical team without delay.

Can a breast lift be done without surgery?

Non-surgical treatments may improve skin quality or the general appearance of the chest area in selected cases, but they do not reposition significantly lowered breast tissue or nipples in the way mastopexy can. Their effect is on the surface rather than on breast support.

Exercise strengthens the pectoral muscles that lie beneath the breasts and can alter the contour of the chest wall, yet it does not tighten stretched breast skin or restore the internal structures affected by ptosis.

Keeping weight stable, avoiding large fluctuations and wearing supportive garments during activity may help limit further change for some people, although they are unlikely to reverse established sagging. The appropriate approach depends on the degree of ptosis, breast volume, skin quality and personal goals.

Mastopexy with auto augmentation

Mastopexy with auto augmentation uses the patient’s own breast tissue, repositioned during the lift, to help shape the upper part of the breast without necessarily placing an implant. Tissue that would otherwise sit low on the breast is secured higher to support the contour.

This option may be discussed when there is enough suitable tissue and the patient wants reshaping with some upper-pole contour rather than a clear increase in size. The degree of fullness achievable depends on the existing tissue, the quality of the skin envelope and the surgical design, and it can settle over time.

Auto augmentation is distinct from other approaches. Fat transfer for upper-pole fullness relies on harvested fat with its own survival and suitability limitations, while implant augmentation adds a device with separate long-term considerations.

No technique can be chosen from photographs or a general description. Suitability for a breast lift is determined through examination and informed discussion of scars, volume goals and future pregnancy plans with Prof. Dr. Şükrü Yazar.

Frequently asked questions

<details class="sy-faq-item">

Can I breastfeed after a breast lift?

Breastfeeding may still be possible after mastopexy, but milk production and milk transfer can be affected when ducts or nerves are altered during nipple repositioning. The likelihood varies with the technique, the degree of ptosis, and individual healing, so future pregnancy and breastfeeding plans should be discussed before surgery.

</details>
<details class="sy-faq-item">

Will stretch marks disappear after a breast lift?

Stretch marks located on skin removed during breast lift surgery may be removed with that skin.
